H1N1 Flu Shots Still Available

The Health Department is still offering the H1N1 flu vaccine throughout the month of February.  The vaccine is available by appointment only.  For more information or to schedule an appointment, call the Pequannock Township Health Department at 973-835-5700 ext. 127 or 109.

Also, a complete listing of other area H1N1 flu clinics is available on the web at www.morrishealth.org. There is no residency requirement for any of these clinics.

SOCIAL EVENTS

All organizations and/or individuals hosting community events in which food and beverages will be served and distributed to the public must be licensed and inspected by the Health Department. At least 2 weeks notice is required. For more information, contact the Florham Park Health Department at 973-410-5319.

ANIMAL CONTROL

Florham Park animal control services are provided by St. Hubert’s Animal Welfare Center located in Madison, NJ.  To report a lost or stray dog or cat, call 973-377-2295.  For after hours emergencies, call the Florham Park Police Department at 973-377-2200.

FERAL CATS
It is very tempting to feed feral cats because they are so cute and look just like domestic cats.  These animals are NOT domesticated.  They have been born in the wild and may carry diseases and in some cases, rabies. The feeding of feral cats and other types of wildlife also disrupts their natural feeding habits and can result in wildlife being concentrated at artificial feeding areas like your yard.  It is against the law in Florham Park to feed feral cats and other wildlife. Violators are subject to prosecution and fines.  Report feral cats to the animal control officer at St. Hubert’s Animal Welfare Center or to the Health Department.

DEAD DEER
Contact the Florham Park Police (973-377-2200) to report any dead deer in the roadway.  If there is a deer carcass located on your property, it is your responsibility to arrange for its removal. This can be accomplished two ways:

1. Contact a carcass removal service to have the carcass removed at your expense.
2. Move the carcass to the roadway and contact the Florham Park Police to arrange for pick-up.

PET LICENSING
All dogs and cats that are at least 7 months old must be counted and licensed each year by the Borough of Florham Park as per Chapter 96 and Chapter 108 of the Code of Florham Park.  Dog and cat licenses must be renewed by January 31st of each year.  N.J. state law requires that all animals must show proof of a current rabies vaccination that is valid through November 1st of the licensing year in order to renew. 2010 Pet License Reminder All previously registered dog and cat licenses are now delinquent and must be renewed immediately.  The fee chart is below:

Dog or Cat (neutered) $27.00
Dog or Cat (non-neutered) $30.00

If your dog or cat is deceased or you no longer have your pet, it is your responsibility to notify the Health Department immediately so that the pet can be removed from the record.  Call 973-410-5319.   Failure to do so can eventually result in enforcement actions for neglecting to license your pet.

 

RABIES AND WHAT YOU SHOULD KNOW
 Pets can become infected with rabies through contact with rabid animals.  Because of this, it is very important that your pet has an up-to-date rabies vaccination.  Florham Park offers a free rabies vaccination clinic each November.  You may also bring your pet to any state/municipally-sponsored rabies clinic in any town or, of course, your veterinarian. Do not feed or handle wild animals and especially avoid contact with all bats, raccoons, skunks, groundhogs, and foxes.  Avoid contact with strays or pets other than your own as these animals my bite or scratch you and may not be vaccinated for rabies.

MOSQUITO CONTROL

The Morris County Mosquito Commission is the lead agency for mosquito control in Morris County.  To report an infestation or a mosquito breeding site, call 973-285-6450.

VITAL STATISTICS

Obtaining a Certified Copy of a Vital Record
The Borough of Florham issues certified copies of birth, death, and marriage records for events that occurred within the Borough of Florham Park.  A certified copy will contain the raised seal of the Borough of Florham Park and can be used for legal or identification purposes.  A certified copy of a vital record can only be issued to those individuals who have direct link to the individual(s) named on the vital record.  The requestor must be able to fully identify the vital record and provide proof of his/her identity and relationship. 

You must complete an application for a certified copy of a vital record.  To download an application, click here.  Also, you must provide the following documentation:

• Self-Identification
  All requests must include your PHOTO driver’s license with address or two alternate forms of ID with your address.  Acceptable types of alternate identification are:  driver’s license without photo, vehicle registration, insurance card, passport, County ID, School ID, utility bill.
  
  
• Proof of Relationship
  You must also include proof of your relationship to the individual(s) listed on the record you are requesting, unless it is your own record. 

Persons requesting records in person must present these documents when filing the application.

Self-Addressed, Stamped Envelope
Mail all requests to the address listed above.  You must include a self-addressed, stamped envelope if you want the record to be mailed back to you.  All mail-in requests will only be mailed to the address shown on your proof of identification. Vital Records are never faxed.

Getting Married or Entering into a Civil Union?
You may make your application to obtain a marriage or civil union license in the town where either applicant resides.  Each town may have their own procedures on the marriage / civil union license process and it is recommended that you contact the local registrar where you will apply for detailed instructions.  If both applicants live out of state, you must apply in the town where the ceremony is to be held in New Jersey.

To Apply for a Marriage License or Civil Union License in Florham Park:
Appointment is required.  Please call the Registrar’s Office at 973-410-5319 to schedule.  It is recommended that you apply at least two weeks before the scheduled ceremony.  There is a mandatory 72 hour waiting period from the time the application is filed to the date the license is issued.  Once issued, the license is only valid for 30 days.

What You Need to Bring When Applying in Florham Park:

• Both applicants and one witness (18+ years old) must be present during the application process.
• The applicants must present valid forms of identification showing your photograph and current address such as a Photo Driver’s License and passport. 
• Applicants’ social security card or know your social security #.
• A birth certificate is recommended but not required.
• If either applicant has been married before, a copy of the final divorce decree showing location and date that it was finalized.
• If either applicant is widowed, a copy of the death certificate of the spouse.
• The application fee is $28.00 payable in cash or check (Borough of Florham Park).

All documents must be in English or accompanied by a certified translation.

The Applicants must also know the following:

• City and state of their birth
• Both parent’s full names/mother’s maiden name
• Parents birth place
• Name, address and phone number of person performing the ceremony 

Your license will be filed with the Registrar of Vital Statistics in the Municipality where the ceremony was performed.

NJ EASE (New Jersey Easy Access, Single Entry) is the easy way for seniors and their families to get information about and access senior services.  NJ EASE is one toll-free number to put you in touch with someone to help you learn about and apply for important programs and benefits.  You will speak to someone who is trained to provide you with answers and information on a wide variety of topics including healthcare, insurance, home care services, long term care options, transportation, social activities, nutrition and more.  Call 1-800-564-4565 (Morris County EASE) for more information.

Cancer Screening Services
Residents who are income eligible and are uninsured or underinsured may qualify for free breast, cervical, colorectal and/or prostate cancer screening services under the NJCEED (Cancer Education and Early Detection) Program.  For more information, call the Morristown Memorial Hospital Breast and Cervical Cancer Screening Program (Morris County) at 973-971-5952.

Prescription Savings Program
Pharmacy Owners Providers Services (POPS) provides all residents of Morris County and dependents in the same house, not currently covered through any government sponsored or employer based prescription insurance program, with savings of up to 50% on prescription medications.  For more information, call 1-800-564-4656.

NJ Family Care
New Jersey FamilyCare is a federally and state funded health insurance program created to help New Jersey's uninsured children and certain low income parents and guardians to have affordable health coverage.  NJ FamilyCare is for families who do not have available or affordable employer insurance and cannot afford to pay the high cost of private health insurance.  For more information and for eligibility requirements, call 1-800-701-0710.
